Staten Island Performing Provider System (SI PPS) is a non-profit public health organization founded in 2014 under the New York State Department of Health Medicaid 1115 waiver amendment known as the Delivery System Reform Incentive Payment (DSRIP) program. Under that program, SI PPS received funding to build a network of medical, behavioral health, and social service agencies tasked with implementing projects to improve health outcomes, improve quality of care, and reduce costs for the Medicaid program.
The Social Care Network connects Medicaid members with community services that address health-related social needs like food, housing, and transportation.
Services include screenings for social needs, connections to community resources, and coordinated support to improve health and well-being.
Individuals can get connected to a Navigator who helps identify needs and link them to local programs and services across Staten Island.
